Personal Data Collected when registering as customer

A certain amount of personal data is collected at the time of  registering for a service or workshop. This is for the purposes of being able to fulfil my work and business commitment to you.

All information gathered by me either on paper or through any online registration process is treated as confidential and will not be shared with a third party. Data that you give is kept securely either in paper form or online and only viewed by Sharon Mayfield, sole proprietor of SharonMayfield.com

You may, of course, decline to submit personally identifiable information to Sharon Mayfield in which case Sharon Mayfield may not be able to provide certain services to you.

 

How I use and store your Health information

When registering for some services with Sharon Mayfield, it is necessary to obtain some of your personal health information in order to offer the most appropriate, beneficial and safe service.  This information will be kept in paper form, stored securely and only viewed by Sharon Mayfield.  This information will not be stored electronically and will never be shared with any third party.  Sharon Mayfield will keep this information for 7 years after your last class as stipulated by my insurance provider.

How I use your Contact Information

When registering for services with Sharon Mayfield you will be asked to sign a disclaimer. In doing so you are agreeing to your email and phone number being used by Sharon Mayfield to inform you of service updates directly relating to the service you have booked.

You may, of course, decline to submit your email and phone number to Sharon Mayfield which in some cases may mean that Sharon Mayfield is not be able to provide certain services to you.

Sharon Mayfield Mailing List

Direct marketing emails informing you of new, future or alternative services I am providing are sent through a secure permission-based email system called MailChimp (see their Privacy Policy at www.mailchimp.com). When you purchase a service from me you will be invited by email to join my MailChimp email list. You have the ability to Opt-In at this point, and subsequently subscription preferences may be changed by you at any time.

Phone Numbers

Sharon Mayfield may, from time to time, use your phone number to contact you about urgent changes to our planned meeting.  This may take the form of a phone call, text or ‘whats app’ message. If you do not wish me to contact you in this way, please inform me at the time of booking your service with me.  If you choose not to be contacted via a phone number in any of these ways, you may not be informed of last minute changes to your service.
